Local Services Tax (LST) Forms
LST FORMS AND INSTRUCTIONS
NEED A NEW TAX ACCOUNT?
You must complete the application for business license in order to establish a new account for the payment of LST. Since this is a business license/registration and it can impact other Township taxes we cannot accept applications from third parties or payroll services. All applications must be submitted/signed by an employee or authorized person from the applying business. The application can be accessed by clicking this link: Application for License.

QUARTERLY TAX FORMS AND INSTRUCTIONS
- 2023 LST QUARTER 3 and 4

EMST/LST Substitute Form (This form is for tax years 2005 and later. It calculates the total amount due you enter the number of employees next to the applicable tax year. It can be used to file multiple years. You may need to open this form in Adobe Reader for the calculations to work. )

OPT Substitute Form 1978 - 2004 (This form calculates the OPT due if you enter the number of employees next to the applicable year.)
- Quarterly Local Services Tax Return (Fillable PDF)
- Adaptable generic form to be used for all quarters before 2021
- May need to be opened in Adobe reader to work

REFUNDS AND EXEMPTIONS
- LST Exemption Certificate
- Fill in the year in the space provided an the top right corner of the form. If you are self employed you will still need to file a quarterly LST return showing no balance due.
- LST Refund Application
- Employees and self employed individuals can use this form to request a refund of LST tax.
