LST FORMS AND INSTRUCTIONS

 

NEED A NEW TAX ACCOUNT?

You must complete the application for business license in order to establish a new account for the payment of LST.  Since this is a business license/registration and it can impact other Township taxes we cannot accept applications from third parties or payroll services.  All applications must be submitted/signed by an employee or authorized person from the applying business.  The application can be accessed by clicking this link:  Application for License.

REFUNDS AND EXEMPTIONS

  • LST Exemption Certificate
    • Fill in the year in the space provided an the top right corner of the form.  If you are self employed you will still need to file a quarterly LST return showing no balance due. 
  • LST Refund Application
    • Employees and self employed individuals can use this form to request a refund of LST tax.
