The below Employment Application link is for current year's seasonal positions in the Parks and Recreation Department.

Employment offers are contingent on the successful completion and return of the below listed items:

Form I-9 - You must present original documents establishing identity and employment eligibility from the List of Acceptable Documents (page 3 of the document) prior to your first day of employment.  Expired documents will not be accepted.  Required Annually.

Form W-4  - Federal Tax Withholdings.  Required Annually.

PA Criminal Background Check  -  Click here to view a summary of the Fair Credit Reporting Act.  Required Annually. 

Local Earned Income Tax Certification - To find your withholding rate by address, click hereRequired Annually. 

Local Service Tax Exemption Certificate (if applicable) - Please complete if you qualify for a Local Service Tax (LST) exemption.  See form for further information.  Required Annually.

Pre-employment Drug Screen - The Chain of Custody Form and List of Approved Collection Facilities will be mailed to you.  Drug Screen results must be received prior to your start date.  Required Annually.

Direct Deposit Form (Optional) - A voided check or bank verification form with account and routing number is required with this form.  Required Annually.

CPSL Disclosure Statement - For both new and returning employees.  Employees that have not received their FBI Fingerprint and/or Child Abuse History certificates must also submit proof that clearances have been submitted prior to start date (ex: receipt, email confirmation, etc).  Required Annually.

PA Child Abuse History Clearance - Required for individuals 14 years of age and older.  If you have a current Child Abuse History clearance that was obtained through another employer within the last 60 months, in lieu of obtaining a new Child Abuse History clearance, you must present the original copy to the Human Resources Department.  To obtain a new Child Abuse History clearance, you must complete an online application or mail a paper application along with a check or money order to the Department of Human Services (DHS).  Please refer to the CPSL Background Check Packet for instructions.  Certificates will be mailed and/or emailed directly to you.  Upon receipt, your original certificate must be presented to the Human Resources Department.  If you are a returning employee and have a current FBI clearance on file with Lower Merion Township, you may skip this section. 

FBI Criminal Background Check - Required for individuals 14 years of age and older.  If you have a current FBI clearance that was obtained through another employer within the last 60 months, in lieu of obtaining a new FBI clearance, you must present the original copy to the Human Resources Department. To obtain a new FBI clearance, you must register online and schedule an appointment prior to going to one of the approved fingerprinting locations.  Please refer to the CPSL Background Check Packet for instructions.  Certificates will be mailed directly to you.  Upon receipt, your original certificate must be presented to the Human Resources Department.  If you are a returning employee and have a current FBI clearance on file with Lower Merion Township, you may skip this section.

Hepatitis B Vaccination Acceptance or Declination Form - Please check your medical records for your vaccination history.  Required Once.

Worker's Compensation Acknowledgement - Please complete this  form to acknowledge that you have received the Worker's Compensation Information and List of Panel PhysiciansRequired Once.
